If you believe that you or someone upon whose behalf you are acting has unclaimed property with Blumont Annuity, you can request a search by submitting the completed Unclaimed Property – Search Request form.

To keep your information as safe as possible, we ask that you send your documents securely by mail rather than email. Please mail them to:

The form asks for information about the group annuity policy and the annuitant. It also asks for ID from the person making the search request. Please make sure to include identification and as much information as possible when you return the form. The annuitant information helps us find the unclaimed property. The ID helps us verify your identity to protect annuitant privacy. If you’re not the annuitant, please include proof that you’re authorized to make this request. This might be a Power of Attorney, estate executor documents or beneficiary documents.

Once we have your form, it will take us about four to six weeks to search our records and get back to you.
